PHH aims to improve the quality of life of a person living with a life threatening illness and supports those receiving palliative care in the home who live within the Mornington Peninsula Shire, City of Frankston and parts of the City of Kingston. PHH offers practical, emotional, psychological, social and spiritual support for the person and their family, including children and young adults.
